Booking, quote and request forms
Request a crew, start a shipment, get a quote, book a shoot: I build the forms, and each one comes in with the details you need, so you’re not chasing them. Every form here is live on a client’s site.
Requests that reach you

Crew requests
Pearls Farm Labor, Napa. Growers send the crop, acres, location and dates in one request, and I can see which page or post sent them.

Shipment requests
Cubic Ship, an independent operator of DHL Express counters. No account: start the shipment online, get a code and walk in. Staff confirm the price before you pay, and real customers send their requests this way.

Booking requests
PhatHead Productions, an Oakland music-video director. Tap a package or a quote line and the booking ticket fills itself in.
Quotes and orders
The Sticker Smith, a print shop in Hayward. I built their store from scratch.

The admin behind them
Where the owner and staff work each request. Most of these screens hold customer details, so I keep them off this site.
- The Sticker Smith adminOrders, quotes and customer records in one place, with a work log in plain English so the owner can see what changed.
- Pearls Farm Labor team viewCrew requests and job applications in separate lists, so a grower’s request doesn’t get buried.
- Counter translator and training simulatorCubic Ship: a customer can type or talk in their own language on their own phone, and the counter reads it in English. New hires can practice realistic shipments, every field checked.
Reviews and referrals
Brian’s Appliance Repair: a reviews page that asks for an honest Google review, with nothing offered for it, and a QR referral card so Brian knows who to thank.
The sites in front of the forms.
Each of these forms sits on a site I built.
So what’s the vision?
Tell me what you do, what you need and when. I’ll write back myself, with what I’d do first.
